บทที่ 20 ข้อปฏิบัติการขอใบอนุญาตทำงาน (Work Permit )

บริษัท สยามราช จำกัด (มหาชน) ดำเนินธุรกิจด้านการบริการงานซ่อมบำรุง งานก่อสร้าง งานติดตั้งต่อเติมระบบท่อ – ถังน้ำมัน – แก็ส และให้คำปรึกษาแนะนำทั่วไป ยึดมั่นในความปลอดภัยในการปฏิบัติงานด้วยระบบใบอนุญาตทำงาน (Work Permit )
ขอกำหนดในการขออนุญาตการทำงานที่จะกล่าวต่อไปนี้ ถือเป็นแนวทางและวิธีปฏิบัติในโรงงานอุสาหกรรม,สถานประกอบกิจการทั่วไป ที่มีวัสดุอุปกรณ์ วัตถุดิบ ผลิตภัณฑ์ที่เป็นเชื้อเพลิง และสารเคมีต่าง ๆ
เป็นองค์ประกอบในกระบวนการผลิต หรืออาจมีสายไฟฟ้าแรงสูง และมีท่อฝังอยู่ในดิน สถานประกอบกิจการ
เหล่านี้จะต้องมีระบบใบอนุญาตทำงานของ พนักงานของ บริษัท สยามราช จำกัด (มหาชน) และผู้รับเหมาจะต้องปฏิบัติตามระบบใบอนุญาตทำงานของลูกค้า หากของลูกค้าไม่มีระบบใบอนุญาตทำงาน หรือมีไม่เพียงพอ ก็เสนอขออนุมัติจากลูกค้าก่อนที่จะใช้ระบบของ บริษัท สยามราช จำกัด (มหาชน)
ก่อนที่พนักงาน บริษัท สยามราช จำกัด (มหาชน) และผู้รับเหมาจะทำงาน จะต้องได้รับอนุญาตจากหัวหน้างานของโรงงานลูกค้าที่รับผิดชอบในอุปกรณ์ หรือพื้นที่ที่จะทำงานก่อน แล้วจึงจะเริ่มทำงานได้ตาม

1. วัตถุประสงค์มีดังนี้
1.1 เพื่อให้ผู้รับผิดชอบต่ออุปกรณ์ หรือบริเวณที่ทำงาน ได้ทราบว่าจะมีการทำงานที่ จุด
ใด เวลาใด เพื่อควบคุมสภาพความปลอดภัย และช่วยดูแลความปลอดภัยให้แก่ทั้งคนงาน
และลูกค้า รวมทั้งให้มีการป้องกันไฟไหม้ด้วย
1.2 เพื่อให้ผู้รับผิดชอบต่ออุปกรณ์หรือบริเวณที่ทำงาน สามารถถอนใบอนุญาตคืน ให้หยุดการ
ทำงานเมื่อมีการปฏิบัติงาน หรือสภาพแวดล้อมเปลี่ยนไปจากสภาวะเดิม อันอาจก่อให้เกิด
อุบัติเหตุ หรือไม่ปลอดภัยเพียงพอสำหรับการทำงานที่ได้รับ อนุญาตให้ทำไปแล้ว
1.3 ผู้รับผิดชอบต่ออุปกรณ์หรือบริเวณที่ทำงาน จะอนุญาตให้ทำงานที่ใหม่ได้ หลังจากที่ถอน
( CANCEL ) ใบอนุญาตแล้ว หรือเมื่อมีการสั่งให้หยุด (HOLD) การทำงานชั่วคราวต่อเมื่อได้
