Hændelse: Under losning af projektl

Hændelse:
Under losning af projektlast, sejlede skibets arbejdsbåd tæt op ad entrepenørens kutter for at planlægge arbejdet.
Vejret var dårligt og det var nødvendigt at gå på fordækket af arbejdsbåden for at kunne snakke med entrepenøren.
Under samtalen dykkede stævnen på skibets arbejdsbåd i søen og den gled hurtigt baglæns.
Besætningsmedlemet på fordækket af arbejdsbåden fik overbalance greb fast i kutterens ræling men kunne ikke klatre ombord på kutteren hvorved denne faldt i vandet
Besætningemedlemmet fik bjerget sig om ombord i kutterens slæbejolle.
Den oppustelig redingsvest blev ikke aktiveret af turen i vandet som følge af den lave vandtempera! tur.

Mulige konsekvenser:
Drukning og underafkøling

Årsag:
Dårligt vejr, besvær med at kommunikere med entrepenøren

Foranstaltninger:
Check af alle arbejdes redningsveste.
Man må ikke gå på fordækket af arbejdsbåden når vejret er hårdt.
Ved de kommende øvelser skal det tydeliggøres at redningsvestene skal udløses manuelt ved lave vandtemperaturer
