1. สำเนาบัตรประจำตัวประชาชน 1 ฉบับ (รับรองสำเนาถูกต้อง พร้อมชื่อผู้รับผลประโยชน์ จำนวน 2 ท่าน)
2. สำเนาทะเบียนบ้าน 1 ฉบับ (รับรองสำเนาถูกต้อง)

ตัวอย่าง ผู้รับผลประโยชน์
1.นาย คนดี สีขาว เกี่ยวข้องเป็น บิดา
2.นาง คนสวย สีขาว เกี่ยวข้องเป็น มารดา



หมายเหตุ ส่งภายใน วันที่ 16 มกราคม 2559 ( หากพนักงานท่านใดไม่ส่งเอกสารภายในวันและเวลาดังกล่าวถือว่าท่านได้สละสิทธิ์ที่จะรับผลประโยชน์โดยไม่มีข้อแม้ใดใดทั้งสิ้น )
