利用者IDの有効期間が残り2週間を切りました。IDの利用期間を延長する

利用者IDの有効期間が残り2週間を切りました。
IDの利用期間を延長する場合は『変更申請』から処理をお願いします。
処理が行われない場合、対象のIDは有効期間満了時に無効化されます。

なお、派遣（委嘱）期間延長予定の方については、下記担当部署にて
延長・変更手続きを行いますので、各自の手続きは不要ですが
必ず下記担当者までご連絡をお願いいたします。
